2021 Andrea Rigler December Freestyle Seminar Registration

All Waivers, releases and acknowledgements will be electronically verified with this registration submittal to limit hand to hand interaction due the current COVID-19 pandemic.

By submitting this form, I hereby acknowledge that I will be participating in the Houston Canine Frisbee Disc Club’s (HCFDC) 2021 Andrea Rigler of Dragon Dogs International (DDI) Freestyle Seminar (Seminar) to be held on December 11 & 12, 2021, which will be held at 11931 Presley Drive, Magnolia, Texas 77354 (venue). I further acknowledge that I will conduct myself in a manner consistent with all DDI and HCFDC rules for sportsmanship and canine safety. Failure to participate in a sportsmanlike manner or to prioritizing canine safety and well-being may result in removal from the Seminar and Seminar venue.

I certify that my canine(s) and myself are healthy, fit, and free from injury to participate in the Seminar and hereby assume the risk of any human or canine disease/injury, which may be contracted at the Seminar. I hold myself personally responsible for any action(s) my dog(s) or I might undertake at this Seminar. I also certify that any dog I bring to the Seminar is current on any legally required vaccinations.

I further agree to comply with all applicable international, Federal, State, and Local COVID-19 related ordinances and guidelines in connection with my participation in the Seminar. I will also require that any family members, friends, helpers, camera persons, etc. attending or, in any way, assisting with my participation in the Seminar abide by the same ordinances and guidelines.

I hereby grant full permission to the HCFDC and DDI the right to record, broadcast, photograph, film, videotape or otherwise make use of any digital recording of our performance(s) and interview(s) at the Seminar and to utilize such recordings, broadcasts, photographs, films or videotapes together with our names, images, likenesses, voices, and biographical information, for such publicity, promotional or commercial purposes as they may desire.

I understand that the HCFDC and DDI may assign the right to use the recordings, broadcasts, images, photographs, film, videotape, or otherwise make use of any digital recording of my performance(s) together with my names, images, likenesses, voices, and biographical information to such person or other entity as they may desire without my further consent or knowledge and I expressly agree to such assignment.

Should I not be able to attend the Seminar, I will notify HCFDC in advance and reimbursement arrangements may be made. Should I have any questions, I will contact the administrator for more information.

I, the undersigned, certify that my canine and I are healthy and fit to participate in the Andrea Rigler of Dragon Dog International Freestyle Seminar [hereinafter, “the Seminar”]. I, the undersigned, intending to be legally bound, do hereby for myself, my heirs, assigns, executors, and administrators waive and release any and all rights for claims and damages for injuries arising out of or in any way related to my participating in the Seminar. Which I may have against the following parties: Andrea Rigler, Dragon Dog International, sponsors of the Seminar, the Houston Canine Frisbee Disc Club, and the administrator and owner of the facility hosting the Seminar, together with contractors, agents, employees, and representatives of each of them.

I do hereby verify, attest, and certify that all the information provided is honest and true to the best of my knowledge.
