2024 Triple Crown & UpDog Games Registration

All Waivers, releases and acknowledgements will be electronically verified with this registration submittal to limit hand to hand interaction due the current COVID-19 pandemic.

By submitting this form, I hereby acknowledge that I will be participating in the 2024 UpDog Challenge Triple Crown & UpDog Games (Event) hosted by the Houston Canine Frisbee Disc Club (HCFDC) and to be held on September 21 & 22, 2024 at 2P Ranch located at 11931 Presley Drive, Magnolia, Texas (Event venue). I further acknowledge that I will conduct myself in a manner consistent with all UpDog Challenge and HCFDC rules for sportsmanship and canine safety. I will also conduct myself in a respectful manner consistent with all rules, regulations, requirements and obligations to the Event Venue. Failure to participate in a respectful or sportsmanlike manner or to prioritizing canine safety and well-being first may result in forfeiture of game scores and/or removal from the Event and Event venue.

I certify that my canine(s) and myself are healthy, fit, and free from injury to participate in the Event and hereby assume the risk of any canine disease/injury, which may be contracted at the Event. I hold myself personally responsible for any action(s) my dog(s) or I might undertake at this Event. I also certify that any dog I bring to the Event is current on any legally required vaccinations.

I further agree to comply with all applicable international, Federal, State, and Local COVID-19 related ordinances and guidelines in connection with my participation in the Event. I will also require that any family members, friends, helpers, camerapersons, etc. attending or, in any way, assisting with my participation in the Event abide by the same ordinances and guidelines.

I hereby grant to UpDog Challenge, Event venue, and the HCFDC the right to record, broadcast, photograph, film, and videotape our performance(s) and interview(s) at the event and to utilize such recordings, broadcasts, photographs, films our videotapes together with our names, images, likenesses, voices, and biographical information, for such publicity, promotional or commercial purposes as they may desire.

I understand that UpDog Challenge, the Event venue, and the HCFDC may assign the right to use the recordings, broadcasts, images, photographs, film, and videotape of my performance(s) together with my names, images, likenesses, voices, and biographical information to such person or other entity as they may desire without my further consent or knowledge and I expressly agree to such assignment.

Should I not be able to attend the event, I will notify the HCFDC administrator, Jill Best, in advance and reimbursement arrangements may be made. Should I have any questions, I will contact the HCFDC administrator for more information.

For and in consideration of my participation in the UpDog Challenge (Events), and for other good and valuable consideration, the receipt and sufficiency of which are hereby acknowledged, I, the undersigned, intending to be legally bound do hereby for myself, my heirs, assignees, executors and administrators, waive and release any and all rights and claims for damages or injuries arising out of, or in any way related to, my participation in the Events(s), which I may have against the following parties: UpDog Challenge Inc., Houston Canine Frisbee Disc Club, the administrator(s), and owner of the hosting facility. I also acknowledge that I have read and agree to abide by the UpDog Challenge General Rules at the time while present at an UpDog Challenge event.
